​If you would like to register with MyMedicare and choose Springvale South Medical Centre as your preferred medical centre, you can now register online or in  person at our clinic.

MyMedicare aims to enhance the relationship you have with your preferred doctore and medical center. By consistently visiting the same clinic and seeing the same GPs for your medical appointments, your chosen doctor can offer you more personalized and continuous care. While you’re free to visit other clinics, you’ll enjoy additional benefits at your selected clinic.

If you’d like to make our medical center your ‘home clinic,’ you’ll need to register with MyMedicare, select our center, and choose your preferred GP. We warmly invite our current and past patients to designate our medical center as their preferred home clinic.

What is MyMedicare ?

MyMedicare is a new way to formalise relationships between patients and their healthcare providers. Seeing your GP regularly and formalising your relationship with them through MyMedicare can lead to better health outcomes.

When you register in MyMedicare and select your preferred healthcare provider, they can get additional government funding to deliver the care you need.

When a patient and their preferred provider are both registered, the benefits include:

• Access to longer Medicare-funded telehealth consultations

• Connections to more appropriate care for people with chronic conditions who visit hospital frequently

• More regular GP visits and better care planning for people living in a residential aged care home

Who Can Register?

 

• You must have a Medicare card or Department of Veterans' Affairs (DVA) Veteran Card.

• You must have had two face to face visits at SSMC in the previous 24 months
